— -- The following are the U.S. military personnel who have been killed in Operation Iraqi Freedom from November, 2004 to December, 2004. All dates reflect when the incidents occurred local time. For deaths before November 2004 and after December 2004, see the links at the left of the page. The lists are compiled from information reported by the Defense Department after notification of family.

Lance Cpl. Jason E. Smith, 21, of Phoenix, Ariz., died as result of hostile action in Al Anbar Province, Iraq.

Sgt. Damien T. Ficek, 26, from Pullman, Wash., died in Baghdad, Iraq, when his patrol was attacked by enemy forces using small arms fire.

Spc. Craig L. Nelson, 21, from Bossier City, La., died at the National Naval Medical Center in Bethesda, Md., of injuries sustained Dec. 16 in Baghdad, Iraq, when an improvised explosive device detonated near his military vehicle.

Pfc. Oscar Sanchez, 19, from Modesto, Calif., died in Mosul, Iraq, when a vehicle-borne improvised explosive device struck his observation post.

Staff Sgt. Jason A. Lehto, 31, of Mount Clemens, Mich., died in a non-hostile incident in Al Anbar Province, Iraq.

Navy Seaman Pablito Pena Briones, Jr., 22, of Anaheim, Calf., died of a non-hostile gun shot wound in Fallujah, Iraq.

Staff Sgt. Nathaniel J. Nyren, 31, from Reston, Va., died in Baghdad, Iraq, when a civilian vehicle struck his military vehicle.

Staff Sgt. Todd D. Olson, 36, from Loyal, Wis., died in the 67th Combat Support Hospital in Tikrit, Iraq from wounds sustained in Samarra, Iraq on Dec. 26, when an improvised explosive device detonated.

Spc. Jose A. Rivera-Serrano, 26, from Mayaquez, Puerto Rico, died in Baghdad, Iraq, from wounds sustained when an improvised explosive device hit his vehicle.

Lance Cpl. Eric Hillenburg, 21, of Marion, Ind., died as result of enemy action in Al Anbar Province, Iraq.

Lance Cpl. James R. Phillips, 21, of Hillsboro, Fla., died as result of enemy action in Al Anbar Province, Iraq.

Cpl. Raleigh C. Smith, 21, of Lincoln, Mont., died as result of enemy action in Al Anbar Province, Iraq.

1st Lt. Christopher W. Barnett, 32, of Baton Rouge, La., died in Baghdad, Iraq, when an improvised explosive device detonated near his military vehicle.

Chief Joel Egan Baldwin, 37, of Arlington, Va., died in Mosul, Iraq, when the dining facility he was in was attacked.

Capt. William W. Jacobsen Jr., 31, of Charlotte, N.C., died in Mosul, Iraq, when a dining facility he was in was attacked.

Sgt. Maj. Robert D. O'Dell, 38, of Manassas, Va., died in Mosul, Iraq, when a dining facility he was in was attacked.

Sgt. 1st Class Paul D. Karpowich, 30, of Bridgeport, Pa., died in Mosul, Iraq, when a dining facility he was in was attacked.

Staff Sgt. Julian S. Melo, 47, of Brooklyn, N.Y., died in Mosul, Iraq, when a dining facility he was in was attacked.

Staff Sgt. Darren D. VanKomen, 33, of Bluefield, W.Va., died in Mosul, Iraq, when a dining facility he was in was attacked.

Spc. Victor A. Martinez, 21, from Bronx, N.Y., died south of Baghdad, Iraq, from a gunshot wound.

Pfc. Christopher S. Adlesperger, 20, of Albuquerque, N.M., died as result of enemy action in Al Anbar Province, Iraq.

1st Lt. Andrew C. Shields, 25, of Campobello, S.C., died in Mosul, Iraq after being involved in an Apache helicopter accident.

Chief Warrant Officer Patrick D. Leach, 39, of Rock Hill, S.C., died in Mosul, Iraq after being involved in an Apache helicopter accident.

Cpl. Kyle J. Renehan, 21, of Oxford, Pa., died in Kaiserslautern, Germany, from injuries received on Nov. 29 as result of enemy action in Babil Province, Iraq.

Sgt. Arthur C. Williams, IV, 31, of Edgewater, Fla., died in Ar Ramadi, Iraq, when his unit was conducting a dismounted patrol and was attacked by small arms fire.

Capt. Mark N. Stubenhofer, 30, of Springfield, Va., died in Baghdad, Iraq, when his unit was conducting dismounted operations and was attacked by small arms fire.

Cpl. In C. Kim, 23, of Warren, Mich., died as result of a non-hostile vehicle incident in Al Anbar Province, Iraq.

Sgt. 1st Class Todd C. Gibbs, 37, of Angelina, Texas, died in Khalidiyah, Iraq, when an improvised explosive device detonated while his unit was on a dismounted patrol.

Staff Sgt. Marvin L. Trost III, 28, of Goshen, Ind., died in Habbaniyah, Iraq, when an improvised explosive device detonated near his HMMWV.

Spc. Edwin W. Roodhouse, 36, of San Jose, Calif., died in Habbaniyah, Iraq, when an improvised explosive device detonated near his HMMWV.

Pfc. Andrew M. Ward, 25, of Kirkland, Wash., died in Ar Ramadi, Iraq, when his unit was attacked by enemy forces using small arms fire.

Staff Sgt. Kyle A. Eggers, 27, of Euless, Texas, died in Habbaniyah, Iraq, when his vehicle was struck by an improvised explosive device.

Cpl. Joseph O. Behnke, 45, of Brooklyn, New York, died in Baghdad, Iraq, when he was in a vehicle accident.

Sgt. Michael L. Boatright, 24, of Whitesboro, Texas died in Baghdad, Iraq, when he was struck by an improvised explosive device.

Staff Sgt. Salamo J. Tuialuuluu, 23, of Pago Pago, American Samoa, died in Mosul, Iraq, when his Stryker military vehicle received enemy fire during convoy operations.

Sgt. David A. Mitts, 24, of Hammond, Ore., died in Mosul, Iraq, when his Stryker military vehicle received enemy fire during convoy operations.

Sgt. Cari A. Gasiewicz, 28, of Depew, N.Y., died in Baqubah, Iraq, when two improvised explosive devices detonated near her convoy.

Cpl. Binh N. Le, 20, of Alexandria, Va., died from injuries received as result of enemy action in Al Anbar Province, Iraq.

Cpl. Matthew A. Wyatt, 21, of Millstadt, Ill., died from injuries received as result of enemy action in Al Anbar Province, Iraq.

Spc. David P. Mahlenbrock, 20, of Maple Shade, N.J., died in Kirkuk, Iraq, when he was clearing a route and an improvised explosive device detonated.

Staff Sgt. Henry E. Irizarry, 38, of Bronx, N.Y., died in Taji, Iraq, when his unit was on patrol and an improvised explosive device detonated near his HMMWV.

Pfc. George D. Harrison, 22, of Knoxville, Tenn., died in Mosul, Iraq, when his HMMWV was attacked by enemy forces using small arms fire.

Cpl. Bryan S. Wilson, 22, of Otterbein, Ind., died as result of a non-hostile vehicle incident in Al Anbar Province, Iraq.

Spc. David M. Fisher, 21, of Green Island, N.Y., died in Baghdad, Iraq, when his unit was on patrol and the HMWWV in which he was riding rolled over.

Cpl Zachary A. Kolda, 23, of Corpus Christi, Texas, died as result of enemy action in Al Anbar Province, Iraq.

Gunnery Sgt. Javier Obleas-Prado Pena, 36, of Falls Church, Va., died at the Landstuhl Regional Medical Center, Germany, from injuries received Nov. 25, as result of enemy action in Al Anbar Province, Iraq.

Sgt. Pablo A. Calderon, 26, of Brooklyn, N.Y., died in Fallujah, Iraq, when an improvised explosive device detonated near his military vehicle.

Sgt. Jose Guereca, Jr., 24, of Missouri City, Texas, died in Fallujah, Iraq, when an improvised explosive device detonated near his military vehicle.

Spc. Daryl A. Davis, 20, of Orlando, Fla., died in Iraq when his HMWWV was involved in a traffic accident.

Spc. Erik W. Hayes, 24, of Cascade, Md., died in Al Miqdadiyah, Iraq, when an improvised explosive device detonated near his military vehicle.

Lance Cpl. Blake A. Magaoay, 20, of Pearl City, Hawaii, died as a result of enemy action in Al Anbar Province, Iraq.

Sgt. Christian P. Engeldrum, 39, of Bronx, N.Y., died in Baghdad, Iraq, when his military vehicle struck an improvised explosive device.

Pfc. Wilfredo F. Urbina, 29, of Baldwin, N.Y., died in Baghdad, Iraq, when his military vehicle struck an improvised explosive device.

Pfc. Stephen C. Benish, 20, of Clark, N.J., died in Ar Ramadi, Iraq, when he received enemy fire while on a dismounted patrol.

Staff Sgt. Michael B. Shackelford, 25, of Grand Junction, Colo., died in Ar Ramadi, Iraq, when their unit was conducting a dismounted patrol and they encountered enemy forces using small arms fire.

Sgt. Carl W. Lee, 23, of Oklahoma City, Okla., died in Ar Ramadi, Iraq, when their unit was conducting a dismounted patrol and they encountered enemy forces using small arms fire.

Spc. Jeremy E. Christensen, 27, of Albuquerque, N.M., died in Ad Duilayah, Iraq, when an improvised explosive device detonated near his patrol vehicle.

Sgt. Michael A. Smith, 24, of Camden, Ark., died at Walter Reed Army Medical Center in Washington, D.C., from injuries sustained in Baghdad, Iraq, on Nov. 7 when he was attacked by enemy forces using small arms fire.

Lance Cpl. Jordan D. Winkler, 19, of Tulsa, Okla., died due to a non-combat related incident at Camp Fallujah, Iraq.

Staff Sgt. Marshall H. Caddy, 27, of Nags Head, N.C., died in Khaladiyah, Iraq, when his military vehicle struck another military vehicle.

Pfc. Jose Ricardo Flores-Mejia, 21, of Santa Clarita, Calif., died in Mosul, Iraq, when an improvised explosive device hit his convoy.

Pfc. Dennis J. Miller, Jr., 21, of La Salle, Mich., died in Ar Ramadi, Iraq, when his unit came under enemy attack and a rocket-propelled grenade struck his M1A1 Abrams tank.

Maj. Horst G. Moore, 38, of San Antonio, Texas, died in Mosul, Iraq, when enemy mortar rounds detonated within his unit living area.

Sgt. David M. Caruso, 25, of Naperville, Ill., died as a result of enemy action in Al Anbar Province, Iraq.

Spc. Travis A. Babbitt, 24, of Uvalde, Texas, died in Baghdad, Iraq, when his patrol was attacked by enemy forces using a rocket-propelled grenade and small arms fire.

Sgt. John B. Trotter, 25, of Marble Falls, Texas, died in Ar Ramadi, Iraq, when he was on patrol and his unit came under attack by enemy forces using small arms fire.

Staff Sgt. Todd R. Cornell, 38, of West Bend, Wis., died in Fallujah, Iraq, when acting in an advisory support capacity and his Iraqi unit came under attack by enemy forces using small arms fire.

Staff Sgt. Russell L. Slay, 28, of Humble, Texas, died as a result of enemy action in Al Anbar Province, Iraq.

Master Sgt. Steven E. Auchman, 37, of Waterloo, N.Y., died from injuries received when multiple rocket-propelled grenades struck his location in Mosul, Iraq.

Cpl. Joshua D. Palmer, 24, of Blandinsville, Ill., died as a result of a non-hostile vehicle incident in Al Anbar Province, Iraq.

Lance Cpl. Thomas J. Zapp, 20, of Houston, Texas, died as a result of enemy action in Al Anbar Province, Iraq.

Spc. Bryan L. Freeman, 31, of Lumberton, N.J., died in Baghdad, Iraq, from wounds sustained earlier that day when he came under small arms fire while conducting a vehicle search.

Cpl. Nathaniel T. Hammond, 24, of Tulsa, Okla., died as a result of enemy action in Babil Province, Iraq.

Lance Cpl. Shane K. O'Donnell, 24, of DeForest, Wisc., died as a result of enemy action in Babil Province, Iraq.

Sgt. 1st Class Otie J. McVey, 53, of Oak Hill, W.V., was medically evacuated from Baghdad, Iraq, on Sept. 23 for treatment of a non-combat related illness. He died Nov. 7 in Beaver, W.V.

Spc. Quoc Binh Tran, 26, of Mission Viejo, Calif., died in Baghdad, Iraq, from wounds sustained that same day when an improvised explosive device detonated near his military vehicle while he was conducting convoy operations.

Spc. Brian K. Baker, 27, of Springville, N.Y., died in Baghdad, Iraq, when a vehicle-borne improvised explosive device detonated near his security patrol.

Pvt. Justin R. Yoemans, 20, of Eufaula, Ala., died at the 31st Combat Support Hospital in Baghdad, Iraq, from wounds sustained earlier that day in Baghdad, Iraq, when a vehicle-borne improvised explosive device detonated near his up-armored HMMWV.

Sgt. Carlos M. Camacho-Rivera, 24, of Carolina, Puerto Rico, died in the 31st Combat Support Hospital in Baghdad, Iraq, from wounds sustained earlier that day in Fallujah, Iraq, from a rocket blast.

Spc. Cody L. Wentz, 21, of Williston, N.D., died in Iraq when an improvised explosive device detonated near his military vehicle while on patrol.
